Social Life 360 is a fully integrated program that is open to all adults, regardless of diagnostic label.
We support adults who are working on building friendships, participating more fully in their communities, developing relationships, and increasing independence. Many of the individuals we work with identify as autistic, ADD, ADHD, or neurodivergent, or they experience challenges such as social anxiety or loneliness.
Our program is aligned with OPWDD requirements for Self-Directed Services and is clearly tied to meaningful, measurable outcomes in a person’s Life Plan.
Social Life 360 is most commonly aligned with Community Habilitation goals under Self Direction.
Our program is designed to support skill-building in areas such as:
Developing and maintaining relationships
Increasing participation in community life
Building communication and social interaction skills
Strengthening independence and confidence in real-world settings
We’re happy to collaborate with your support broker and FI to determine the best structure.
Social Life 360 is a structured, skills-based coaching program that teaches adults how to navigate social situations with greater confidence and independence.
Our approach follows a structured process of learning, practicing, and applying skills in real-world situations.
We focus on:
Initiating and sustaining conversations
Understanding and responding to social cues
Building reciprocal relationships over time
Navigating social environments (events, groups, dating, friendships)
Following up and maintaining momentum in relationships
We break these skills and strategies down into formulas, diagrams and clear step-by-step instructions, all designed to encourage real-world practice and use.
In addition to learning these skills and strategies, participants are encouraged to complete structured exercises designed to help them build mastery and apply what they are learning in real-world social situations.
Social Life 360 can support a wide range of Life Plan outcomes, including:
Increasing meaningful social connections
Reducing isolation and loneliness
Participating more actively in community settings
Building confidence in social and communication skills
Developing greater independence in forming and maintaining relationships
All services are goal-oriented and aligned with individualized outcomes identified by the person and their team.
Our 16-week program includes:
1:1 Coaching Sessions
Individualized, goal-oriented support with our team that focuses on building and applying social and communication skills and strategies. Sessions are tailored to each participant’s needs and support progress toward increased independence, confidence, and community participation.
Weekly Group Coaching Sessions
A structured “Community Call” for our clients to ask social questions, engage with our community, practice skills and strategies, share their social wins, and learn alongside their peers
Step-by-Step Training Modules
Short, structured lessons delivered in a voice-over-text format, accessible anytime from anywhere, using formulas, diagrams, checklists, and step-by-step processes to support skill development and real-world application
Skill-Building Exercises Designed for Real-World Application
Participants are encouraged to complete structured exercises that reinforce skill development and support measurable progress over time. These exercises encourage practice and real-world application in social environments, helping participants build independence and confidence in forming and maintaining connections and relationships.
Strategic Action Plan
A guided, fill-in document that helps you organize what you’re learning, apply it in real world social settings, and track your progress. Our coaching team has access to it and can provide direct feedback to support your growth.
Unlimited Coaching and Support
Email our coaching team between sessions for personalized feedback and guidance as you apply what you’re learning in real life, so you can ask for support anytime, anywhere.
Community-based online discussion groups
Interest-based and location-based discussion groups where our clients and graduates engage with peers, practice their skills and strategies, and build connections
While coaching is delivered virtually, the skills being developed are designed for real-world use in the individual’s community.
Social Life 360 is open to all adults who want to build social confidence and develop meaningful relationships, and is not diagnosis-specific.
While some of our clients have a formal diagnosis of autism, ADD, or ADHD, others self-identify as neurodivergent or simply experience challenges such as social anxiety, loneliness, or difficulty integrating into their community, or are looking for more structure, support, and clear strategies to build social and relationship skills.
